CLI: refine inline code styling

- Dark theme: inline code uses #fa8329 on #374151
- Light theme: inline code uses #fa8329 on #f3f4f6
- Honor palette without overriding inlineCodeFg in renderers
- Keep fenced code readable across themes
- Update tests to match new defaults
