[Bug] tool_call_id 重复错误 - 多轮对话中 MCP 工具调用导致 400 错误

问题描述

当对话历史中包含多次 MCP 工具调用（如记忆系统的 search_memory、add_message）时，继续对话会触发 DeepSeek API 返回 400 BadRequestError，提示 Duplicate value for 'tool_call_id'。
该问题导致多轮对话无法正常进行，用户必须频繁开启新会话才能继续使用。

复现步骤

基础条件

1. 使用 DeepSeek (deepseek/deepseek-chat) 作为 LLM 提供商
2. 配置 MCP 服务（至少一个，如 memos 记忆系统）

复现流程

1. 开启新的 QQ 群聊会话
2. 发送消息触发 MCP 工具调用（至少 2-3 次），例如：
   • "我叫什么名字？" → 触发 search_memory
   • "帮我记住我喜欢看书" → 触发 add_message
   • "我刚才说喜欢什么？" → 再次触发 search_memory
3. 继续正常对话 5-15 轮
4. 当对话历史累积到一定程度（约 18+ 条消息）后，下一次 LLM 请求会触发错误

关键观察

• 错误发生时，对话历史中往往包含多次相同的工具调用
• 错误提示 message[18] 表明是第 18 条消息存在问题
• 问题与上下文长度相关，但不一定是简单的长度问题

额外观察

1. 问题与 Issue [Bug] Context truncation breaks function call/tool response pairing, causing Gemini 400 error #5416 相似：都是 ContextTruncator 截断消息时破坏 tool_calls 配对
2. 切换 LLM 无效：尝试切换到 Kimi moonshot 模型同样会触发该问题
3. 清空会话无效：删除 data_v4.db 后重启，问题仍然出现
4. 禁用部分 MCP 延后问题：禁用 bing 等服务可以延后问题发生，但无法根本解决

根本原因：原先的 ContextTruncator.fix_messages() 方法只做单向检查——移除孤立的 tool 消息，但不检查包含 tool_calls 的 assistant 消息后是否有对应的 tool 响应。当截断发生在两者之间时，会导致消息序列不合法，DeepSeek API 因此报 Duplicate value for 'tool_call_id' 错误。

修复方案：PR #5417 重写了 fix_messages() 方法，使用状态机确保工具调用/响应配对要么一起保留，要么一起丢弃。

建议：

1. 请升级到 v4.21 或更高版本（如果 PR fix: ensure tool call/response pairing in context truncation #5417 已包含在后续版本中）
2. 如果已是最新版本仍有问题，请在此 Issue 补充更多信息，可能存在其他未覆盖的场景

另外，Issue #1150 是另一个 MCP 工具重复调用的问题（v3.5.1），维护者 @Soulter 已确认修复，但与你的问题不完全相同。
