This PR adds an Asp.Net Core instrumentation by listening to the DiagnosticsSource event generated by the framework.

This instrumentation traces all requests processed by Asp.Net Core and records unhandled exceptions as errors. The created spans are activated, meaning that all spans created inside a request will be parented to it automatically.

The native part of the managed loader writes some debug logs.
Before this change, it was constructing the string to be logged even if the debug log was not enabled.
This change is a performance improvement that makes sure that debug log lines are only created when necessary.

## Summary of changes

Prevent deadlock betwen signal-based profilers (walltime/manual cpu
profilers) and non-signal based profilers (exception, contention....)

## Reason for change

When an exception occurs, the thread can be interrupted by a
signal-based profiler (walltime/manual cpu). It can be interrupted while
holding the lock used to update the `dl-iterate-phdr` cache.
