Number of columns are insufficient in .defect files

Hi Prof. Murphy, I am having an error, "Error : Number of columns insufficient for --"
Could you please let me know how to address it? Thank you.

This is an error related to the .defects file. Each defect you are investigated is assigned a row, with the columns in each row described in the manual. The number of columns needed is dependent on the material being studied: for example, if investigating MgO, a column is needed stating the number of Mg added/removed to make the defect, with a second doing the same for O.
