Riot.im was rebranded to Element in July 2020. Updated the chat links to use the universal matrix.to format which works with any Matrix client. Changes: - README.md: Update 'Riot channels' → 'Element channels' and riot.im → matrix.to - grants/grants.md: Same updates for consistency
