Update outdated Riot.im links to Element/Matrix.to

Summary

Updates outdated chat platform references from Riot.im to the modern Matrix.to format.

Background

Riot.im was rebranded to Element in July 2020. The current documentation still references the deprecated riot.im URLs which may confuse new contributors looking to join the community chat channels.

Changes

  • Updated "Riot channels" → "Element channels" in the Real-time conversation section
  • Replaced https://riot.im/app/#/room/... links with https://matrix.to/#/... format
  • Applied changes to both README.md and grants/grants.md for consistency

Why matrix.to?

The matrix.to URL format is the universal Matrix link format that:

  • Works with any Matrix client (Element, FluffyChat, Nheko, etc.)
  • Allows users to choose their preferred client
  • Is the officially recommended way to share Matrix room links
