UI and Backend - Report Tool Box

Create a slide down report tool box from main NAV. This would have the defaults selected:
Example selections could be:
Equipment Retired - No bug filed for this yet but a toggle for equipment we don't want showing up in default reports.
Equipment Status -
Equipment Location - return only results for Users Default location (see bug: #21)
Equipment Type - return only that type

Stretch Goal
Return matching Ports

This would make default reporting faster. (we certainly don't need to return retired unless we want it)

ideas?
Front End
Could do multi-selects for Status, Type, and DC.
Check box for Retired (left unchecked)

This would also solve the DC vlan report (see bug: #16)

Back end
Set defaults unless something is chosen to keep the reports consistent with current except "retired", that can be a default.

A lot of the back end is working, although, I may refactor what I have, by moving the work to the report options drop down, I can make changes without trying to explain it to users.

Look at this to remove undefined values from form submission.

Consider having the main search field take comma separated array, starts with and ends with.
