Fixed description bug introduced in NodeNorm v2.3.27 (#346)

Restores `id.description` (accidentally moved to a top-level
`description` field in PR #339) and creates a new top-level
`descriptions` field that includes a list of unique descriptions in the
same order as in `equivalent_identifiers`.

Closes #345
