Claude Code have broken permissions management for background agents

Due to bug in latest version of Claude Code. Background agents constantly run in issues with tool permissions and halicunate, reporting score 5.0.

Add requirement for SDD plugin commands to

• use foreground agents only
• Do not accept score 5.0 as halicunation
• Do not accept results if it respond too long report (it means that it not used scratchbook)
• Do not accept results that missing score (it means that it wasn't able to read instructions)

Following commands is affected:

• /sdd:plan
• /sdd:implement
