load_dataset should complain about overlapping sample+locus+replicate combinations

I just inadvertently provided asamples.csv with duplicated Locus/Sample/Replicate combinations across rows, and CHIIMP accepted it with no complaints until reshaping some of the final output tables during the report prep. At the very least it should complain about this clearly when loading the spreadsheet at the start.
