first project save is sometimes empty

When trying to run wp project save for the first time in an existing site, the config is empty.

Note: running wp project save again works as expected.

Looks like it's being caused by Config::default, see here and here.

Basically, when constructed, the config class sets its default property based on whether or not the file exists (true if it doesn't), and the save method only gets the actual data to save if default is false.

Is there a reason to check for the default property there?

I believe you are correct, the check for default is a little too early in the command. The original goal of default was to avoid issues on an entirely blank project init, but when WordPress is actually installed (even if default), yeah, we'd want to get the live configuration values.
