OpenRouter Provider: GPT-5.2-Codex missing xhigh variant (only low/medium/high)

Description

When using the OpenRouter provider with GPT-5.2-Codex, the xhigh variant is not available. Only low, medium, and high appear.

Steps to reproduce

1. Configure OpenCode to use the OpenRouter provider.
2. Select model OpenRouter > GPT-5.2-Codex.
3. Use ctrl+t to swap variants.
4. Observe available variants with xhigh missing.
