revert and unrevert commands break File Changes in Session Summary

Using the revert and unrevert commands messes up the Session Summary, in the video it demonstrates

Steps to reproduce

1. Prompt for a file creation in one message
2. Prompt for another file creation in another message
3. Revert the last message
4. Notice that second file that was "reverted" is the only one in the file summary for the session

The Session Summary file list corruption you're describing seems to be a specific manifestation of the revert functionality bug.
