File view does not auto-refresh after AI modifies the file

Description

Expected behavior: The file view should automatically refresh and display the updated content after the AI modifies it.
Actual behavior: The file view continues to show the pre-modification content. A manual re-open is required to see the changes.

1. Open a file inside OpenCode (TUI or Web UI)
2. Ask the AI to modify that file
3. Observe the file view still shows the old content
