Closes #258

Origin: Discussion #140 by @msquibb

Audit and refresh Teams MCP documentation per issue #258:

- Replace deprecated O365 Connector setup with Teams Workflows
(Power Automate) webhooks — Connectors were retired by Microsoft
- Update webhook URL examples from outlook.webhook.office.com to
  Workflows-format URLs (prod-XX.logic.azure.com)
- Add 'What you need to know' section clarifying Squad does not ship
a Teams MCP server — you bring your own
- Fix mcp.md sample prompt to link to notifications guide instead of
  overpromising built-in Teams guidance
- Keep community gist reference (verified still live and working)
