Summary

Adds a Weekly Retrospective ceremony with Ralph enforcement and a Model Review ceremony to the Squad ceremonies template. Closes #601.

Action items -> GitHub Issues labeled retro-action (not markdown checkboxes)
Includes a deep retro template for thorough weekly reviews

Model Review - Quarterly + event-triggered model evaluation ceremony:

Runs quarterly or when major model releases are announced
Reviews agent/model assignments for cost vs quality tradeoffs

Why this matters

The problem: Scheduled retros get skipped. Markdown action items have ~0% completion rate without enforcement.
The solution: Ralph enforces the retro schedule. If Test-RetroOverdue() returns true, the retro runs before the board is checked.
