Migrate coverage reporting from Coveralls to Codecov

Replaces Coveralls with Codecov for coverage reporting, removing the php-coveralls dependency in favor of the codecov/codecov-action GitHub Action.

Changes required

1. .github/workflows/ci.yml

In the phpunit job, make the following changes:

• The Setup PHP step already uses coverage: pcov — leave that unchanged.

• In the "Run PHPUnit with coverage" step, change the clover output path from build/logs/clover.xml to clover.xml (and remove the mkdir -p build/logs line):

Note: the current "Run PHPUnit with coverage" step has a bug: mkdir -p build/logs in the run block — remove that line and update the clover path.
