[NGO Admin] [Form builder] 2 fields are missing from scale type question

Summary:
Scale question lacks the 2 fields that are supposed to set the labels for the upper and lower label of the scale. (for example - 1 is for very bad, maximum number is for very good). These labels should be configurable by the NGOs.

Steps to Reproduce:

1. Open or create a form
2. Create a scale question type
3. See the difference between figma and the actual scale question
