feat: Model Profiles (quality/balanced/budget)

Summary

Single --profile quality|balanced|budget flag on orchestration commands that maps agent roles to appropriate models (e.g., Planner→opus/opus/sonnet, Explorer→sonnet/haiku/haiku) with per-agent overrides. Enables cost control for teams.

Context

Identified in both competitive analyses (#107, #114). Combines model routing (recommending the right model) with profile-based configuration (setting it per-command).
