[BUG] Plan Viewer "Key Lookup" not showing right icon/text

Describe the Bug

When I open an execution plan that contains a "Key Lookup" operator, it shows as a "Clustered Index Seek" both the icon and the text.

Steps to Reproduce

1. Open "Plan Viewer"
2. Open a .sqlplan file /paste the XML/drag & drop a plan that contains a "Key Lookup"

Expected Behavior

Show the right icon/text.

"Plan Viewer" should show:

Fixed by PR #413 — overrides PhysicalOp/LogicalOp/IconName when Lookup=true to show Key Lookup (Clustered) or RID Lookup (Heap).
