test: add coverage for UpgradeError and SeerError classes

• Add tests for UpgradeError class (5 tests covering all reason types and custom messages)
• Add tests for SeerError class (9 tests covering all reason types and format() behavior with/without orgSlug)

These two error classes were previously untested, contributing to the low coverage (5.94%) for src/lib/errors.ts.
