Create and Update docs for tracker, plan mode and TODO list

Hi! I'd like to work on this issue.
After exploring the codebase, I can see that docs/cli/plan-mode.md and docs/tools/todos.md already exist, but there's no documentation for the tracker feature yet (trackerService.ts, trackerTools.ts).

My plan:

• Create docs/tools/tracker.md covering the 6 tracker tools (tracker_create_task, tracker_update_task, tracker_get_task, tracker_list_tasks, tracker_add_dependency, tracker_visualize) from packages/core/src/tools/trackerTools.ts
• Review and update docs/cli/plan-mode.md and docs/tools/todos.md if anything is outdated

A few questions before I start:

1. Is docs/tools/tracker.md the right location for the tracker doc?
2. Are there specific changes in the plan-mode and todos docs you'd want to be updated, or is the tracker doc the main deliverable?
