What did you expect to happen?

I expected the plan displayed to me in the cli to match the current prompt, and not the one of the plan before it.

Looks like the previous plan wasn't deleted and the model may've hallucinated. Strange that exit plan mode was called with a past plan.

It looks like we need to explicitly delete the plan file rather than have the model try to do it to avoid cases like this. Just to confirm, did this happen in the same session?
