An example of a name resolver returning both IPv4 and IPv6 endpoints for each server. Otherwise like the nameresolver example.
changed NameResolver example to use a target string directly instead of constructing it and changed LoadBalanceServer to have an array of ports instead of serverCount and startPort with logic everywhere.
