Create a form for HfLA organization to Verify Volunteer

Overview

Verifying volunteer hours has become a significant time investment for Bonnie and the organization. To streamline this process, we propose creating a form for members who are leaving project teams or HfLA entirely. This form will provide Bonnie with the necessary information to efficiently respond to employer requests for verification.
