Phase 5: Stale Obligation Nudge + Bilateral Deadlock Escape
PR #12 fixed post-evidence bilateral ghost. Phase 5 fixes the pre-evidence residual.

Phase 5B: Unilateral evidence escape (72h evidence_submitted, counterparty ghost)
