Can you provide a blurb on what the purpose of this PR is, and what it's supposed to change/introduce?

This PR introduces VsCode debugging to DTR (all the other REMS repos also have their own related PRs) for both local runs as well as for the docker dev environment. As part of that work I also updated the docker images a bit, mainly changing the working directory to make source path mappings easier to work with. Also the bin/ directory was updated to include auto registering to the test-ehr so that doesn't need to happen as part of the workflow steps anymore (this was done in HL7-DaVinci and then merged into this branch as well)
