modifies the bundle to accommodate changes for REMS as defined in the new compliance bundle. In general, the changes to the bundle will more than likely come in the form of modifications to the QuestionnaireResponse. The current approach is to rely on the Questionnaire to include information, or attachments if needed, rather than add more custom parts to the bundle.
The key difference here is what DTR and CRD is responsible for, and what the user is responsible for. If DTR wants extra information, it either needs to collect it from the EHR using CQL, or it needs to be given it through the appContext when being launched.
This PR makes minimal changes in order to avoid complicating the process.
