Reopening file does not update

When opening an existing file, saving the file does not update the TOC section. You must remove and re-insert the TOC. Would be a great addition to have TOC auto update files.

I've created PR 10 ( #10 ) to address this.

PR #10 was kinda broken, backed it out until I have time to fix it.

In the hopes that the upstream was still maintaining, I submitted a PR there too - nok/markdown-toc#113

will try to port it back over here when i have a little more time
