Issue with the agent handoff sample - Something went wrong , we need to start over

The new code base that is in github that we deployed today is giving the above issue. The additional information that we got from application insights is 'Turn error: Cannot read property 'addAgent' of undefined"

The previous version of code is working fine for us,

Any help or pointers here please will be of great help

On further analysis, ( In the agent view) what we found is activity invocation is failing with 502, token and Conversation is successful but failing in activity. (Browser Logs). In the app server logs it gives HTTP Error 404.0 - Not Found
