Update dashboarding links to Grafana

The production instance of grafana is now available at:

https://grafana.telemetry.mozilla.org/

Let's update the live monitoring dashboard links to point to that and populate:

experiment_id={slug}
from={start timestamp}
to={now}

as query params

We can't do this until after we've completed the PRD for monitoring so this is blocked for now.
