Incorporate issue dedupe

When giving the summarize or default prompt for an issue, there should be a part that attempts to de-duplicate the issue against any others in the repo, providing the LLM a list of issues with similar titles or descriptions for the LLM to decide if they're similar enough to count as duplicates.
