When the selected primary test root is . (repo-root tests), this scope check drops every direct match because files like bar.test.js are neither equal to . nor prefixed with ./. In that layout, a changed source file with an existing matching root-level test produces no scoped matches and /codex:test fails with “Unable to infer test targets…”, even though the correct update target exists.

The scoped-file filter has the same . root handling problem here, so if the chosen root is repo-level (.), root tests (for example foo.test.mjs) are excluded from extension counting. That makes the extension inference fall back to .js, causing /codex:test to create *.test.js in repositories whose root test convention is *.test.mjs or *.test.ts.
