Parse issue form on issue creation

An issue-form parser currently exists but is limited

• https://github.com/stefanbuck/github-issue-parser - right now can't compile checkbox entries into an array
• https://github.com/peter-murray/issue-forms-body-parser - don't use entry id for field key

The idea might be to contribute to one of these project and use our fork containing the changes we need until the plugin is updated.
