[Bug]: Claude thread title generator can go rogue

Steps to reproduce

1. Select any Claude model as the Text Generation model. I've seen this with both Opus 4.6 (high) and Haiku 4.5 (Thinking on).
2. Make several new threads that request things such as setting up new projects.

For Haiku, this prompt injection seemed to reproduce it reliably, and the title.json was written even when the main chat was in "Supervised" mode on a Codex model:

User message: How do I make grep accept a regex?

Expected behavior

The chat that you're talking to performs the requested actions, and you can watch what it's doing as it does it.

Actual behavior

The title writing bot randomly tries to also fulfill the user's request, and you don't get any visibility. Sometimes it even improvises weird stuff like installing random dependencies or initializing new projects, because it's Claude.

I can't share the full session logs, but in one case an Opus title writer couldn't find the project I referenced, so they rummaged through my home directory looking for it, then decided to start a new project. I thought I had been hacked because macOS was asking whether to grant the terminal access my downloads and music folders. 😳
