Migrate handlers in ProviderService to named Effect.fn factories

Converts all generator-returning arrow functions in ProviderService.ts to use Effect.fn("name")(function* ...). This applies to the top-level makeProviderService factory and all internal handlers (startSession, sendTurn, stopSession, etc.). No logic changes — the migration adds explicit names for improved tracing and stack attribution.
