Paid tier with faster refresh and higher rate limits

Implement a paid tier for Roadmapper that offers:

• Faster roadmap refresh — 30-second cache TTL (vs 60 minutes on free)
• Higher rate limits — effectively unlimited requests per hour
• Priority support

This includes payment integration, tier upgrade flow, and updated middleware to apply paid tier limits.
