fix: worktree branch tracking and workspace naming #2

- Fetch remote-tracking refs after bare clone so origin/* branches are available
- Set upstream tracking on worktree branches so git pull/git push work out of the box
- Use repo name (from remote URL) for workspace file naming instead of directory name
