Command palette button added multiple times

On Ubuntu 18.04 Firefox 62.0 (64-bit) while navigating through code command palette button added multiple times for each visited file.

It looks like it can be reproduced in any repository. For example, in this repository:

1. Open repository page https://github.com/sourcegraph/browser-extensions - no command palette button.
2. Open .editorconfig - button added.
3. Go back and open .gitignore - another button added.
4. Go back and open .travis.yml - the third button added.
