I added v-mode feature.

  • Add Visual mode support: line-based actions use the selected range (path:10-20 / #L10-L20).
  • Fix GitHub URL path resolution to be repo-root relative and normalize separators; update README.
  • Backward compatible; normal-mode behavior unchanged.
