feat: harden operator interventions and bulk review UX

Summary

• tighten the operator intervention surfaces in Mission Control and Needs Attention
• add clearer bulk review trays for decision and intervention selection in the narrow right rail
• surface a primary decision handoff in blocked slice detail, including a fallback path when the slice only emits decision counts
• keep grouped scope sections and related mission-control detail surfaces aligned with the improved operator workflow
