Create a partially covering index idx_gw_by_originator_cover on gateway_envelopes to speed up API queries

Add migrations that create and drop a covering index on gateway_envelopes.

• Create idx_gw_by_originator_cover on (originator_node_id, originator_sequence_id, gateway_time) including topic with statement_timeout set to 0 in 00019_add_gateway_index.up.sql
• Drop idx_gw_by_originator_cover if it exists in 00019_add_gateway_index.down.sql
