Add PR #45 to add support for the IncludeOnline parameter on accounts receivable invoices.
Added parseBooleans on the xml2js parser so booleans are automatically parsed. This removes the need for redundant string conversions.
Note: This is potentially a breaking change, so bumping version to 2.2.0 after this merge.
