Add usage examples for ext packages #608

With the move to separate distributions for ext packages, many of our packages have "TBD" in their README files. See e.g. https://pypi.org/project/opencensus-ext-zipkin/.
@reyang added some docs in #541. This issue is for the rest of the packages.
