This is a housekeeping PR for #535 and #540.
This is part of the effort for #445.

  1. Moved extension specific samples to the extension folder.
  2. Removed wrapt, psycopg2 and grpcio dependency from core opencensus.
  3. Moved extension specific document to the extension folder.
  4. Added a list of existing extensions to the main README file.
  5. Added badge for README: add OpenCensus pypi version badge #519.
