This is part of an alternative to #1244, breaking it down into smaller PRs for review.

The usual things such as capitalization of the name section after the
dash, and consistent "Usage" handling in modified files.

Note that all changes to PGML.pl are reverted.  There is no POD, and so
there should not be a `=head1 NAME` section added (and an empty `=head1
DESCRIPTION section to boot), and all other changes were just comment
clean up.

Rather than adding all of the comments here, I am just adding a pull request to fix the usual things. Note that all changes to PGML.pl will be reverted in that pull request. There is no POD, and so there should not be a =head1 NAME section added (and an empty `=head1 DESCRIPTION section to boot), and all other changes were just comment clean up.
