add per-pool backend stats

Parse the output of SHOW pool_pools and try to assemble useful counts of backend pool connections by user/db/backend.

This obviously leaves us in a situation where both pool_processes and pool_pools are processed with special-case logic, but I could not immediately think of any better way to do it. Suggestions welcome!
